Knocking and juddering.. :(

Right ill try explain this simply.. I think it's my engine mounts but to be fair I have no idea..
my gearstick has always been pretty wobbly when I accelerate but over a year it's got worse, sometimes it jumps out of 4th, usually only when I go over a massive bump cause my ride is very harsh!
When I accelerate, usually in 1st 2nd and 3rd I can get up to a certain amount of revs then let off and the gearstick goes mental like wobbling, and it sounds like the engine is wobbling about..

help.. Is milo gonna die?

Dog bone mount? (Rear engine mount)

Would that cause this? That's what I thought it was, engine/gearbox mount, will order one now if it might be that!
