SQL / Excel question

I have a SQL Query taking data from 5 tables (might not matter). The data is currently shown as

User id, time, form id, question, answer
1234, 12:00, 9999, What colour?, Blue
1234, 12:00, 9999, What shape?, Square
1234, 12:00, 9999, Where?, Floor 12
1234, 12:00, 9999, What flavour?, Sour
1234, 12:00, 9999, Who did it?, HRM

I would like it shown as:

User ID, Time, Form ID, What Colour?, What Shape?, Where?, What Flavour?, Who did it?

1234, 12:00, 9999, Blue, Square, 12, Sour, HRM

Does anybody know how to make this happen? I have never used SQL before and have spend the morning going through 14 tables to try and locate the data I need, now I just need to format it as I need it!
