Replaced shock problem

My brother has replaced his rear shocks on his 182 Trophy with Koni adjustables and he encountered a problem as shown in the photos, can anyone tell me why this has happened? I haven't had chance to actually look at the car yet to see if I can work it out.

It looks like the lower cup washer hasn't been fitted underneath- which has allowed the piston to push through the bush?
