Yes you have to rotate it twice anticlockwise to equalise the tension across the belt.
I checked and double checked when previously building the engine back up when it was out the car and it was fine so no idea what happened.
The noise was a metallic rattling noise heard when I blip the throttle on the second half of the video.
Turns out the timing was wrong and the inlet cam had moved out of position. Strange seeing as I did the timing when the engine was out and checked it 4 or 5 times.
Have redone the timing...
Right I've tried all I can to eliminate this misfire and horrible noise but I still have no idea what can be causing it.
Things I have changed:
Injectors
Spark Plugs
Leads
Coil Pack
Cam position sensor
It also has a brand new Dephaser.

Yeah, I'm sure I've bent a valve on cylinder 1 when the cam slipped round.
When I was taking it apart on the inlet manifold for cylinder 1 there was black liquid stains which I assume were petrol coming out the inlet.
Next step is to take the head off...
Did a compression test this morning.
0 on cylinder 1
120 on cylinder 2
didn't do 3 or 4 as there was no point.
Stripped it down bled all the tappets put it back together and it still sounds like an absolute dog :/

No, unfortunately I don't have a compression tester but with retrospect when I was turning the crank it was FAR too easy to do. I was doing it with a small 1/2 drive rachet.
Ok, well checked my HT leads and as I suspected they are on the correct way, tried a bit of WD40 still no luck.
Going to have a go at the lifers in the morning as I'm pretty certain I've timed it up correctly.
Tom once you've drained the lifers do you just put them straight back in or do you...
That sounds like it could be possible. Thinking back it didn't feel as their was much compression when I was cranking the engine to check timing....

I tensioned it up with all pulleys loose, tightened, turned it over twice tool slipped in lovely.
Everything was torqued up correctly, this is my 4th belt change on a F4R and neither have sounded like this before :\
